Just guessing here, but maybe your front page is displaying “all”, not “subscribed”? lemmy.world would be linked with lemmynsfw, so this post can show up in “all”.
You can also disable NSFW from your settings and re-enable if/when you want to see NSFW stuff.

Why is this posted as if it’s onnlemmy.world when it’s from lemmynsfw.com?
I’ve blocked lemmynsfw on this account to avoid poem on my feed randomly when I don’t want it.
