There’s a great reason for that: this is a Holly Randall picture and Holly learned her trade from her mum, Suze Randall, who pretty much invented this kind of lighting and post-processing in the 90s. Apparently the challenge was to something artistic enough to keep the models happy and to stand out from the crowd while also keeping Larry Flynt happy by making sure that the sets were explicit and filthy enough.
